THIS INSTRUCTION LIST IS FOR A SELF-DON (FRONT-ENTRY) DUI DRYSUIT

At the top of the form, make sure your name is clearly written on the form as your name will be written on the inside of the drysuit. Your dealer will write their dealership name as well as the date the measurements are taken.
